LPN Trach & Vent Care Specialist for Home Care | Middleburg Heights, Ohio
About The Position
BrightStar Care is seeking a skilled Licensed Practical Nurse (LPN) with Trach & Vent experience to deliver high-quality, specialized patient care in a home care setting. The role involves providing one-on-one patient care, focusing on airway management and critical care. Requirements
- Active Ohio RN license (unencumbered)
- 1+ year clinical experience
- Strong clinical and independent decision-making skills
- BLS/CPR certified + TB clearance
- Reliable transportation & valid driver's license
- Ability to lift up to 50 lbs and assist with patient mobility
- Up-to-date vaccinations (COVID-19, Flu, Hep B)

- Home care/critical care experience preferred
Responsibilities
- Provide trach and ventilator care in patients' homes under RN supervision
- Assist in implementing individualized care plans and follow care instructions
- Monitor patient condition and promptly report changes to the RN or Director of Nursing
- Maintain accurate, timely documentation in the home care setting
- Follow agency protocols, safety standards, and HIPAA compliance
- Educate patients and families on home trach and ventilator care
- Collaborate with the home care team to support optimal patient outcomes
- Participate in ongoing training and professional development
- Provide skilled trach and ventilator care within RN scope of practice
- Follow and implement individualized care plans using clinical judgment
