About The Position

What You Should Know About the LPN Team Float Role: This is a DRIVING position and will travel to see patient in both homes and facilities within the greater Dayton area This is a FT position, day shift 8:00am-4:30pm with an every 6th weekend rotation and 1 holiday per year We provide superior care and superior services to patients at their end of life journey. Only those who have a heart for hospice will succeed The LPN Team Nurse Essential Duties: The LPN Team Float provides quality nursing care to patients in a manner that meets the patient and family needs working under the direction and supervision of an RN Care Manager.

Requirements

LPN's - Registered LPN with the State of Ohio without any board actions. One year of previous nurse experience.
CPR Certified
Computer skills sufficient to properly document services and care.
Ability to drive during daytime, nighttime or inclement weather.
Valid Driver's License with Safe Driving Record
State Minimum Automobile Insurance Coverage

Nice To Haves

Certification in Hospice & Palliative Medicine (CHLPN) preferred; required within two years of employment. (Eligible upon completion of 500 hours in last 12 months of hospice/palliative experience.)
