About The Position
What You Should Know About the LPN Team Float Role: This is a DRIVING position and will travel to see patient in both homes and facilities within the Champaign and Logan Counties and surrounding areas This is a FT position, M-F 8:00A-5:00P We provide superior care and superior services to patients at their end of life journey. Only those who have a heart for hospice will succeed The LPN/RN Float Essential Duties: The LPN Team Float provides quality nursing care to patients in a manner that meets the patient and family needs working under the direction and supervision of an RN Care Manager.
Requirements
- LPN's - Registered LPN with the State of Ohio without any board actions. One year of previous nurse experience.
- CPR Certified
- Certification in Hospice & Palliative Medicine (CHLPN) preferred; required within two years of employment. (Eligible upon completion of 500 hours in last 12 months of hospice/palliative experience.)
- Computer skills sufficient to properly document services and care.
- Ability to drive during daytime, nighttime or inclement weather.
- Valid Driver's License with Safe Driving Record
- State Minimum Automobile Insurance Coverage
