About The Position
This position offers skilled, one-on-one nursing care for pediatric patients in their homes, providing an alternative to facility-based work. Nurses will enjoy consistent schedules with long-term cases, allowing them to build meaningful relationships with patients and families. Responsibilities include administering and managing medications, monitoring and documenting patient conditions, performing physician-ordered treatments, and educating and supporting family members. The role emphasizes compassion, integrity, and unwavering support, aiming to create new beginnings for both patients and nurses.
Requirements
- Active IL RN or LPN license (or Compact license)
- Strong assessment and communication skills
- Compassionate, dependable, and self-motivated
- Valid driver’s license and reliable transportation
Responsibilities
- Provide skilled, one-on-one nursing care in the home
- Administer and manage medications
- Monitor and document patient conditions
- Perform treatments as ordered by the physician
- Educate and support family members
- Collaborate closely with the Phoenix clinical team
