About The Position
Phoenix Home Care & Hospice is seeking LPNs and RNs for pediatric home care, offering a unique opportunity to provide skilled, one-on-one nursing care to patients in their homes. This role is designed to avoid the chaos of facility work, providing consistent schedules with long-term cases. Requirements
- Active IL RN or LPN license (or Compact license)
- Strong assessment and communication skills
- Compassionate, dependable, and self-motivated
- Valid driver’s license and reliable transportation
Responsibilities
- Provide skilled, one-on-one nursing care in the home
- Administer and manage medications
- Monitor and document patient conditions
- Perform treatments as ordered by the physician
- Educate and support family members
- Collaborate closely with the Phoenix clinical team
