About The Position

Angels of Care Home Health is seeking Licensed Practical Nurses (LPN) in Columbia, SC and surrounding areas to provide direct, one-on-one medical care to medically fragile infants, children, and/or adolescents primarily in their home. Angels of Care has been serving this community since 2000, operating on values of Heart, Advocacy, Love, Outreach, and Speed (HALOS). The role involves focusing on patient medical needs and treatment according to the active plan of care, conducting ongoing assessments, administering medications and therapies, coordinating care, educating family members, and implementing preventative initiatives to ensure quality care. Requirements

• Active LPN/LVN license
• Provide care in a client home setting
• Ability to make a positive and lasting impression!
• 1 year experience required

Responsibilities

• Focus on the medical needs and treatment of infants, children, and/or adolescents primarily in their home according to the active plan of care, as given by the physician and other appropriate personnel.
• Conduct on-going patient care and assessments.
• Administration of prescribed medication, treatments, and therapies.
• Coordination of care
• Educate family members on patient clinical care to enhance positive outcomes
• Preventative initiatives to protect quality of care for patient
