Requirements

Graduation from an accredited school of practical nursing is required.
One (1) year of nursing experience is required; home care experience is preferred.
Current LPN Licensure is required in the Commonwealth of Massachusetts and may be required in Rhode Island, subject to geographic area assignment.
Basic Life Support (BLS) & CPR certification is also required.
Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired

Nice To Haves

Proficient computer skills, including experience with various software, database and electronic records programs is preferred.
home care experience is preferred.

Responsibilities

Under the direction of the Clinical Manager or designee, provide competent health care to individuals and families in the home setting.
Promote health maintenance of individuals and families through teaching, counseling, collaborative planning and appropriate preventative and rehabilitative nursing measures.
