Requirements

  • Graduation from an accredited school of practical nursing is required.
  • One (1) year of nursing experience is required; home care experience is preferred.
  • Current LPN Licensure is required in the Commonwealth of Massachusetts and may be required in Rhode Island, subject to geographic area assignment.
  • Basic Life Support (BLS) & CPR certification is also required.
  • Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired

Nice To Haves

  • Proficient computer skills, including experience with various software, database and electronic records programs is preferred.
  • home care experience is preferred.

Responsibilities

  • Under the direction of the Clinical Manager or designee, provide competent health care to individuals and families in the home setting.
  • Promote health maintenance of individuals and families through teaching, counseling, collaborative planning and appropriate preventative and rehabilitative nursing measures.
