Requirements

Active LPN/LVN license in the state of practice
Passion for wound care and patient-focused healthcare
Strong communication and organizational skills
Ability to work independently in a mobile setting
Valid driver’s license and reliable transportation

Responsibilities

Provide advanced wound care therapies, including low-frequency, non-contact ultrasound (MIST)
Travel locally to patients’ homes while following strict safety and infection control standards
Collaborate with an interdisciplinary care team to ensure excellent outcomes
Educate patients and families on wound care and recovery
Complete accurate, timely documentation in the EMR
Grow professionally through ongoing training and advancement opportunities
