General Summary The Licensed Practical Nurse (LPN) assesses patient and implements nursing care plan as ordered. Participates in coordination of care with other disciplines the patient is assigned. Participates in call rotation as assigned.

Requirements

  • Graduate of an accredited LPN program and approved IV therapy course.
  • LPN license Missouri or compact state.
  • American Heart Association Basic Life Support certification.
  • Valid Driver’s license and proof of insurance.
  • Considerable mental concentration required.
  • Lifting up to 35 lbs., turning activities and nearly constant walking required.
  • Standing, turning, carrying, pushing, pulling, stooping, crouching, twisting, and reaching.
  • Frequent exposure to communicable disease via blood and body fluids. Frequent exposure to infectious diseases and hostile persons at times. Works with sharps and other potentially biohazardous materials. Variation in environmental temperatures. Exposure to various community environments.

Nice To Haves

  • One year home health care experience preferred.

Responsibilities

  • Evaluates patient for continuing to meet eligibility requirements for home health per CMS or payer guidelines and agency policy.
  • Assesses patient accurately reporting any change in patient condition promptly to an RN.
  • Implements all procedures/treatments per policy and procedure, following infection control guidelines.
  • Completes all documentation within specified time frames.
  • Organizes and provides nursing care in a cost efficient manner including scheduling and utilization of visits and patient medical supplies.
  • Collaborates with other disciplines assigned to patient. Documents care coordination in patient medical record.
  • Assists with development of discharge plans.
  • Assists with development of plans for patient/caregiver education with focus on patient self-management. Evaluates effectiveness of the plan. Individualizes methods of learning and teaching techniques to each patient as appropriate. Give feedback to the RN as indicated regarding patient's progress with education and retention of information.
  • Appropriately manages time and daily tasks to achieve daily productivity goals.
  • All other duties as assigned within scope of practice.
  • Reflects on performance by turning mistakes and successes into learning opportunities.
