As a Home Health LPN, you will work closely with a team of healthcare professionals to ensure that our clients receive the best possible care tailored to their individual needs. You will carry out nursing procedures, monitor patient progress, and provide education and support to patients and their families. Requirements

  • Current LPN license in the state of practice
  • Minimum of one year of nursing experience, preferably in home health or community health
  • Compassionate and patient-centered approach to care
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Basic computer skills and familiarity with electronic health records
  • Willingness to undergo background checks and health screenings

Responsibilities

  • Provide direct patient care in accordance with individualized care plans
  • Monitor and record patient vital signs and health status
  • Administer medications and treatments as prescribed by a physician
  • Educate patients and families on health management and care procedures
  • Collaborate with healthcare team members to optimize patient care
  • Assist with daily living activities such as bathing, dressing, and meal preparation
  • Document patient care activities in accordance with company policies and healthcare regulations
