About The Position

We’re looking for a We want YOU to join our exceptional team as an LPN Clinical Coordinator to join or weekend team who’s flexible, dependable, and passionate about quality care. You’ll assess resident needs, support care plan implementation, and provide compassionate clinical care. Lead and mentor CNAs while helping the team deliver consistent, person-centered care every day.

Requirements

Current South Carolina LPN license in good standing.
Compassionate, patient, and empathetic personality.
Strong communication, leadership, and problem-solving skills.
Ability to work professionally with residents, families, staff, and physicians.

Nice To Haves

Long-term care experience preferred but not required.

Responsibilities

Assess resident needs
Support care plan implementation
Provide compassionate clinical care
Lead and mentor CNAs
Support the Director of Nursing in leading and overseeing the Nursing Department
Work closely and collaborate with the DON and ADON to maintain consistency of workflow within the unit
Provide hands-on resident care while supervising daily clinical operations
Ensure compliance with state, federal, and company regulations
Monitor documentation, physician orders, labs, and care coordination
Support staffing, performance oversight, and quality improvement efforts
Serve as a liaison between nursing staff, physicians, residents, and families
Participate in audits, care plan meetings, infection control, and safety initiatives
Take part in the administrative on-call rotation as assigned
Deliver top-notch nursing care to our residents
Provide leadership and guidance for our nursing staff
