The RN Case Manager manages a caseload of patients and families, which includes assessing, planning, implementing, evaluating and documenting the care provided. Why Choose Santè? Requirements

  • Graduate of an accredited school of nursing.
  • Active licensure in good standing as a Registered Nurse through the State Board of Nursing.
  • Minimum of one (1) years’ nursing experience in hospice or home health care preferred.
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order and a clean driving record.
  • Current CPR and Fingerprint clearance or ability to obtain

Nice To Haves

  • CHPN certification a plus

Responsibilities

  • Identify risks and interventions upon admission, and coordinate with other team members to help address those concerns.
  • Administer medications and treatments as prescribed by the physician.
  • Be available, when possible, to meet a patient/family's needs in time of crisis.
  • Attend and participate in bi-weekly Interdisciplinary Team Meetings (IDT), and provide recommendations on revising the plan of care based on continued evaluation of the patient and family/caregiver needs.
