Requirements
- Graduate of an accredited school of nursing.
- Active licensure in good standing as a Registered Nurse through the State Board of Nursing.
- Minimum of one (1) years’ nursing experience in hospice or home health care preferred.
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order and a clean driving record.
- Current CPR and Fingerprint clearance or ability to obtain
Nice To Haves
- CHPN certification a plus
Responsibilities
- Identify risks and interventions upon admission, and coordinate with other team members to help address those concerns.
- Administer medications and treatments as prescribed by the physician.
- Be available, when possible, to meet a patient/family's needs in time of crisis.
- Attend and participate in bi-weekly Interdisciplinary Team Meetings (IDT), and provide recommendations on revising the plan of care based on continued evaluation of the patient and family/caregiver needs.
