About The Position

This position, under the direction of the Director of Nursing and RN Nurse Manager, assists in the evaluation of available resources to assure quality outcomes of care. They also assist in the direction and implementation of the nursing process. This includes assisting the Director of Nursing and RN Nurse Manager with systematic data collection; assist with the development of care plan, and implementation of care plan and revision of plan. The Care Coordinator, LPN coordinates the management of the care of residents through a well-defined care delivery system. Requirements

  • Completion of a professional nursing education program approved by the Minnesota Board of Nursing.
  • Current Minnesota License as a Practical Nurse.
  • Current Certification in Cardiopulmonary Resuscitation (CPR) and emergency care or be willing to get one upon hire.

Nice To Haves

  • Experience in skilled nursing and/or assisted living facilities.
