Keystone Health is seeking a full time LPN Care Coordinator to join the team. The LPN Care Coordinator will partner with the RN Care Managers to manage a defined cohort of patients within the Chronic Care Management (CCM) program. Care Coordinators assist with establishing patient goals and providing virtual clinical support to improve patient outcomes and quality of life. Care Coordinators serve as a patient point of contact and as an information resource for the patient, the care team, and the family. Requirements

  • Current Licensed Practical Nurse license.
  • Knowledge of Patient Centered Medical Home initiative or willingness to learn.
  • Minimum of two (2) years working in a clinical setting.
  • Valid LPN license.
  • Current CPR certification.
  • Strong communication and critical thinking skills
  • Well organized
  • Able to work both independently and in a team setting.
  • Excellent written, verbal and listening communication abilities.

Nice To Haves

  • Experience in care management
  • Strong skills with Microsoft Office and Excel
  • Bilingual skills

Responsibilities
