LPN Care Coordinator

Requirements
- Current Minnesota LPN license required
- Strong communication skills with the ability to work professionally and respectfully with staff, residents, and families
- Must pass a DHS background study and meet the physical requirements of the position
Nice To Haves
- Assisted Living experience highly preferred
- Familiarity with Alzheimer's and dementia care is a plus
- Confidence using electronic documentation systems
- Team-oriented mindset and desire to contribute to a supportive work culture
Responsibilities
- Provide hands-on nursing care including medication administration, treatments, and support with activities of daily living (ADLs)
- Work closely with the RN and leadership team to support individualized care plans
- Complete documentation accurately and in a timely manner
- Support Resident Assistants through mentorship, coaching, and positive leadership
- Manage medication orders, refills, and medication processes
- Perform supervisory visits and complete chart reviews
- Ensure compliance with company policies and Medicare Comprehensive License requirements
- Communicate effectively with residents, families, and team members
- Assist with ongoing quality assurance and regulatory compliance
- Other duties as assigned
