Requirements

  • Current Minnesota LPN license required
  • Strong communication skills with the ability to work professionally and respectfully with staff, residents, and families
  • Must pass a DHS background study and meet the physical requirements of the position

Nice To Haves

  • Assisted Living experience highly preferred
  • Familiarity with Alzheimer's and dementia care is a plus
  • Confidence using electronic documentation systems
  • Team-oriented mindset and desire to contribute to a supportive work culture

Responsibilities

  • Provide hands-on nursing care including medication administration, treatments, and support with activities of daily living (ADLs)
  • Work closely with the RN and leadership team to support individualized care plans
  • Complete documentation accurately and in a timely manner
  • Support Resident Assistants through mentorship, coaching, and positive leadership
  • Manage medication orders, refills, and medication processes
  • Perform supervisory visits and complete chart reviews
  • Ensure compliance with company policies and Medicare Comprehensive License requirements
  • Communicate effectively with residents, families, and team members
  • Assist with ongoing quality assurance and regulatory compliance
  • Other duties as assigned
