About The Position

Trinity Springs, a Cassia community, is hiring Licensed Practical Nurses (LPNs) to join our team in Oxford, FL as a Memory Care Coordinator. At Trinity Springs, you'll be part of a supportive, mission-driven team that takes pride in making a difference every day for residents and families alike. In this role, you are responsible for providing high quality clinical care and services to residents consistent with regulations and established clinical best practices. You will help our residents experience independence and dignity within a loving community atmosphere. We are seeking passionate and dedicated individuals who want to make a difference in our residents’ lives. Position Type: Full-Time, benefits eligible position working a varying schedule including a weekend rotation

Requirements

  • Must have a current Florida LPN license and be in good standing.
  • Knowledge of FL assisted living AHCA regulations.
  • Must have strong communication and organizational skills.
  • Self-starter with a desire to be a part of a team.

Nice To Haves

  • Experience in Assisted Living preferred.

Responsibilities

  • Display a courteous attitude and respect for all residents, families, and staff.
  • Coordinate, oversee, and provide resident-related care for assisted living and memory care residents.
  • Administer medications and/or treatments in accordance with physician's orders and monitors for effectiveness/response.
  • Manage medications, supplies, and equipment including ordering, receiving, storing, and disposing of all items in accordance with policies and procedures.
  • Document care using computerized medication administration and charting software
  • Notify Clinical Manager, attending physicians, and family members of significant changes in a resident's condition, and takes follow up action as necessary.
  • Monitor nursing personnel on duty during the assigned shift.
