The LPN Admissions Nurse is responsible for completing comprehensive start-of-care assessments for patients referred to home health or hospice services. This role evaluates patient eligibility, reviews clinical pieces of referral, obtains verbal CTI’s, reviews documentation for accurate coding requirements, and ensures compliance with Medicare Conditions of Participation, Washington State regulations, and agency policies. The Clinical Admissions Nurse serves as a critical clinical gatekeeper and liaison between referral sources, physicians, and the interdisciplinary care team. Find your sense of belonging at Wesley! Requirements

  • Current Licensed Practical Nurse (LPN) license in Washington State or out-of-state MSL
  • If MSL, follow the Washington Nurse Practice Act.
  • If MSL, complete Suicide Prevention Training in advance of hire per state regulations.
  • If MSL, complete Washington State Board of Nursing (WABON) MSL demographic survey in advance of hire per state regulations.
  • Experience with admissions or case management.
  • Ability to work independently in community settings
  • Strong clinical assessment and critical-thinking skills
  • Excellent written and verbal communication skills
  • Time management and organizational skills
  • Cultural competence and sensitivity to diverse populations
  • Able to lift 25lbs on a regular basis.
  • Ability to use a computer keyboard or touch screen to document resident records.
  • Ability to listen to and comprehend information and ideas provided through conversation, email, and text.
  • Ability to focus and concentrate throughout shift.

Responsibilities

  • Eligibility & Care Appropriateness Review referrals to determine home health eligibility, including skilled need and homebound status
  • Validate physician orders, diagnoses, and supporting clinical documentation
  • Identify patients who may require a higher or alternative level of care and escalate concerns appropriately
  • Plan of Care & Coordination Coordinate services with physical therapy, occupational therapy, speech therapy, medical social work, and home health aides
  • Provide clear clinical handoff to the admission and case-managing nurse
  • Patient & Caregiver Education Educate patients and caregivers on diagnoses, medications, symptom management, safety, infection prevention, and emergency procedures
  • Assess patient and caregiver learning readiness and support systems
  • Promote patient rights, informed consent, and self-determination
  • Documentation & Regulatory Compliance Ensure all documentation meets: Medicare Conditions of Participation (42 CFR Part 484) Washington State home health licensing rules (WAC 246-335) Agency policies and payer requirements
  • Obtain required consents, notices, and acknowledgments (e.g., patient rights, advance directives, financial responsibility)
  • Maintain accurate, timely, and legally defensible clinical records
  • Communication & Collaboration Communicate effectively with physicians, referral sources, intake staff, and clinical leadership
  • Participate in interdisciplinary case conferences as required
  • Serve as a professional representative of the agency in the community
  • Communicate with coding team on appropriate admissions documentation and questions
  • Quality, Safety & Professional Practice Identify high-risk patients and initiate appropriate interventions or escalation
  • Comply with infection control, patient safety, and emergency preparedness standards
  • Maintain patient confidentiality in accordance with HIPAA and Washington State privacy laws
  • Participate in quality improvement initiatives and required trainings
