Reliant at Home is seeking a Full-Time Home Health RN Admissions Nurse in the Reliant at Home Plano coverage area. Must have HCHB experience and strong documentation skills. Requirements

Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
Current State license as a Registered Nurse, current State Driver's License.
Two years experience as a Registered Nurse in a clinical care setting and 2-3 years Full-Time Home Health experience.
Reliable transportation. Valid and current auto liability insurance. Current CPR.

Responsibilities

Primary functions are to perform Hospice admissions in the patient home setting.
Develop and document the plan of care in correlation with the assessment.
Provide skilled nursing care as ordered by physician.
Timely submission of all required documentation.
Communicate with supervisor, interdisciplinary team, patients/ families, and physician in the coordination of care process.
Demonstrate commitment, professional growth, and competency.
Meet productivity standards.
