This role is critical to ensuring a smooth transition, safe care delivery, and exceptional resident and family experience during admission, stay, and discharge. As a Lotus Nurse | LPN , you are responsible for coordinating and supporting the resident experience from admission through discharge, ensuring clinical accuracy, education, and continuity of care. Within the first 24 hours of admission, you will introduce yourself to the resident and their representative, ensure required medical equipment is present in the room, review discharge medication lists, obtain and verify prescriptions, ensure first doses of medications are available, and provide education on medication schedules, PRN use, call light usage, meals, therapy, activities, staffing, side rails, and consents including psychotropic medications, vaccines, and bed rails. You will also provide admission materials, contact information, and ensure the resident and representative understand the facility’s capabilities and care processes. You will attend and participate in the 48-hour care plan meeting, complete admission surveys, review upcoming appointments, and ensure care plans accurately reflect resident needs. Throughout the stay, you will serve as a point of contact for residents and representatives, addressing questions and coordinating care with interdisciplinary teams. At discharge, you will ensure resident and family education is completed related to ongoing care needs such as insulin administration, catheter care, and wound care. You will collaborate with Social Services to address discharge planning, barriers, and assessments; review discharge documentation with the resident for signature; coordinate medications with the home pharmacy; ensure prescriptions are provided; collaborate with home health agencies regarding post-discharge care needs; and complete a 72-hour follow-up call to support a safe transition. All duties are performed in accordance with state Nurse Practice Acts, facility policies, and state and federal regulations, with a strong emphasis on resident satisfaction, communication, and care continuity.
