About The Position

The Program Coordinator supports the development, coordination, and operational performance of the LifeCare Memory Partners respite program service lines. This role functions at a high administrative level and is responsible for assisting with program planning, scheduling, and administrative oversight to ensure efficient delivery of the respite services. Key responsibilities include managing care team and patient scheduling workflows, ensuring timely completion of documentation, assisting with quality and performance metrics, and supporting program reporting and operational projects. In addition, the Program Coordinator provides broader administrative support to the Memory Partners Program Director and interdisciplinary team. This includes supporting intake processes, facilitating communication among internal and external partners, assisting with documentation and compliance requirements, and helping coordinate initiatives across the program. The Program Coordinator contributes to program development through effective organization, communication, and administrative leadership.

Requirements

  • Education: Bachelor’s degree required. Preferred fields: Health Services, Social Sciences, Public Administration, Business, or related disciplines.
  • Experience: Minimum of one year of experience in a health-related field, home care services, program coordination, or advanced administrative support preferred. Knowledge of Alzheimer’s disease, dementia care, or aging services is desirable. Demonstrated ability to manage data, create and format correspondence, organize projects, and prioritize tasks.
  • Essential Technical / Motor Skills: High proficiency with Microsoft Office, especially Outlook, Word, Excel, and PowerPoint. Ability to operate standard office equipment (computer, phone systems, copier, fax, etc.). Experience with scheduling systems, database management, or program tracking tools preferred. Strong orgatizational skills, attention to detail, and problem-solving abilities.
  • Interpersonal Skills: Strong written and verbal communication skills; tactful and professional in all interactions. Ability to interpret requests, manage competing priorities, and maintain positive relationships across all levels of the organization. Demonstrated commitment to customer service and caregiver/client support.
  • Essential Physical Requirements: Stooping, bending, sitting for long periods, and occasional lifting of up to 25 pounds. Ability to move throughout the office and support occasional off-site meetings or errands.
  • Essential Mental Abilities: Strong concentration, ability to multitask, and skill in prioritizing work in a dynamic environment with frequent interruptions. Ability to apply judgment, follow complex procedures, learn new systems, and manage confidential information with discretion.
  • Essential Sensory Requirements: Ability to see and speak clearly to perform job functions and communicate effectively.

  • Hours of Work: 40 hours per week; schedule may include occasional evening or weekend hours based on program needs.
  • Driver’s License: Valid driver’s license and reliable transportation required.
  • Population Served: Same populations as the LifeCare Memory Partners program (individuals and caregivers impacted by dementia).

Responsibilities

  • Manages and maintains scheduling workflows for respite services, ensuring efficient coordination of visits, supervisory requirements, and caseload needs.
  • Ensures all respite regulations and licensure requirements are met and maintain compliance.
  • Assists with monitoring respite performance metrics, maintaining program dashboards or spreadsheets, and preparing reports for internal and external stakeholders.
  • Supports project-based work, including the development of new processes, policies, or documentation to strengthen the respite service line.
  • Implements processes, plans and initiatives to insure service quality and program growth and efficiency.
  • Coordinates communication between respite staff, caregivers, clients, referral partners, and internal departments to ensure seamless operations.
  • Assists with intake and onboarding processes for new clients and caregivers; coordinates required documentation and information gathering.
  • Serves as the primary contact for inquiries about program services and routes referrals appropriately.
  • Performs high-level administrative and clerical tasks including document preparation, compiling records, managing correspondence, and organizing program files.
  • Maintains calendars, monitors the program email inbox, and performs follow-up communication to support client and caregiver engagement.
  • Tracks program activities such as support groups, community education events, outreach activities, and volunteer involvement.
  • Assists with preparation for meetings, special events, and presentations, including room setup, materials preparation, and logistics support.
  • Helps ensure adherence to local, state, and federal grant requirements by maintaining accurate documentation and supporting reporting cycles.
  • Adjusts daily and weekly workflows to support program goals and respond to changes in operational demand.
  • Demonstrates positive, professional communication and customer service with caregivers, clients, staff, and community partners.
