As a Life Enrichment Director, you will assess, document, and maintain patient/resident information related to care plans, MDS, participation records, progress notes, and reviews. complete initial activity assessments for new admissions, evaluating physical and cognitive abilities for participation in programs. participate in care plan conferences, clinical meetings, and interdisciplinary team meetings as required. plan, lead, and assist with group activities and patient/resident committee meetings; maintain required documentation and monthly meeting minutes. monitor, evaluate, and improve Life Enrichment programs to ensure quality, effectiveness, and resident engagement. coordinate with other departments and volunteers to deliver ongoing, high-quality programs that meet patient/resident needs. maintain confidentiality and protect patient/resident rights in accordance with organizational and regulatory standards. respond appropriately to emergency situations and changing resident needs. communicate effectively with patients/residents, families, staff, volunteers,

Nice To Haves

  • must have valid driver's license
  • experience in Life Enrichment and / or working in an assisted living residence or skilled nursing center
  • experience as a CNA or resident assistant is preferred but not required
  • experience with memory care residents

Responsibilities
