About The Position

Approximately 10% of all dialysis patients reside in a SNF and are transported to dialysis three times a week placing a significant burden on patients and adding significant avoidable costs to the health care system. DSD teammates have access to all the capabilities and resources DaVita has to offer. Teammates must be comfortable working autonomously and have demonstrated the ability to collaborate with cross-functional resources across the organization. The primary work address is DaVita's 6-chair SNF dialysis den on-site at Casa Azul 1480 Katy Flewellen Rd, Katy, TX 77494. The LVN may need to provide coverage at other local DaVita SNF dialysis or ICHD facilities. DaVita is seeking a Licensed Vocational Nurse who is looking to give life in a skilled nursing facility. Requirements

  • Dialysis Experience required
  • Current LPN-LVN license in the state of practice required
  • Current CPR certification required
  • IV certification required if required by state regulations
  • High school diploma or equivalent required
  • Completed training program approved by the medical director and the governing body as required by CMS guidelines prior to operating the water treatment system
  • Basic computer skills and proficiency in MS Word and Outlook required; functional proficiency in computer systems of DaVita within 90 days of hire required)

Nice To Haves

  • BONENT or CHT certification preferred or as required by state regulation
  • Minimum of one year's experience in a hospital or clinical setting preferred
