The Hospice (LVN) Licensed Vocational Nurse will work under the supervision of a Registered Nurse Case Manager and provide skilled patient care as outlined in the written plan of care. By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.

Requirements

Completion of a Licensed Vocational Nursing Education Program from an accredited school.
Minimum of one year of LVN experience preferably in an acute care setting.
Current Licensed Vocational Nurse certification from the California Board of Vocational Nurse and Psychiatric Technician Examiners.

Nice To Haves

Experience in a home health/ hospice setting preferred but not required.
Valid California certification in Intravenous Therapy & Blood Withdrawal preferred

Responsibilities

Assist a primary nurse in providing direct care.
Initiate communication with physicians and team leaders, IPG members, HHA’s and other health care providers as needed to facilitate optimal care.
Instruct primary caregivers, patients, volunteers and employed caregivers in care provision based on the Plan of Care and as requested by the Primary Nurse or RN Team Leader. Note: The LVN receives direction from the RN on-site and may not alter the patient/family plan of care.
Attend patient conference and participate in discussion of issues to facilitate optimal care
