About The Position
Bella Terra Hospice is part of the Cornerstone Group, a network of over 75 home health and hospice agencies nationwide. They operate as a close-knit local team with a mission to provide compassionate, high-quality care that enhances the lives of patients and their families. The company is growing and seeking dedicated, experienced LVNs to join their team to build the best hospice agency in the Inland Empire. As an On Call LVN, the role involves supporting patients during nighttime hours, working closely with RNs and physicians to ensure continuity of care and comfort for patients.
Requirements
- Valid California LVN license
- Minimum 2 years of direct patient care experience
- Current CPR certification
- Reliable transportation with valid driver’s license and insurance
Nice To Haves
- At least 1 year of hospice experience preferred
Responsibilities
- Follow and implement the patient’s plan of care as directed by the RN
- Provide timely and accurate documentation
- Conduct assessments and report findings to the care team
- Collaborate with interdisciplinary team members
- Use medical equipment and supplies efficiently
- Engage in ongoing professional development
