Requirements
- Graduate of an accredited nursing program and active Licensed Vocational Nurse (LVN) license in California
- Minimum of 1 year of hospice nursing experience
- CPR certification, valid driver's license, reliable transportation, and valid auto insurance policy
- Knowledge of state and federal home health regulations
- Clinical proficiency in nursing specialty, strong communication skills and compassion
Responsibilities
- Provide home-based care to patients with a prognosis of six months or less to live, focused on quality of life
- Work as part of an interdisciplinary team comprised of a physician, RN, CNA, HHA, social worker, chaplain and volunteers to carry out the patient’s plan of care
- Perform nursing services which are planned, delegated, and supervised by the registered nurse
- Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care
- Take a holistic approach to care that engages the patient’s mind, body, spirit and family
