About The Position

We are currently hiring experienced and dedicated Licensed Vocational Nurses (LVNs) seeking flexible per diem nursing jobs in home health care. Requirements
- Current and active California LVN license in good standing
- Graduate of an accredited vocational nursing program
- Minimum 1 year of nursing experience (home health experience preferred)
- Reliable transportation and valid California driver’s license
- Physical ability to assist and move patients as needed
- Strong communication and documentation skills
- Proficiency with electronic charting and care coordination software

Responsibilities
- Provide skilled, patient-centered nursing care based on individualized care plans
- Accurately document all visits, assessments, and treatments using electronic health records (EHR)
- Communicate effectively with patients, families, and the home health care team
- Ensure compliance with California home health regulations and company policies
