About The Position

Under the supervision of the Healthcare for the Homeless Project Director, the Licensed Therapist, as part of the Primary care team, is responsible for delivering comprehensive mental health and Substance Use Disorder (SUD) services. This position will deliver evidence-based, trauma-informed therapeutic interventions to support recovery and overall wellness.

Requirements

  • Master’s degree in Social Work, Counseling, Psychology, or a related Behavioral Health Professional field
  • Licensed clinical social worker (LCSW); a licensed marriage and family therapist (LMFT); a licensed professional clinical counselor (LPCC); a licensed psychological practitioner (LPP); a licensed psychologist; a licensed professional art therapist (LPAT); or those with a similar qualifying license
  • Must be licensed in the state of Kentucky
  • Strong communication skills with the ability to build rapport with clients from diverse backgrounds
  • Knowledge of psychotherapy techniques and group therapy facilitation is essential
  • Ability to work collaboratively within a multidisciplinary team
  • Ability to provide therapy services independently without supervision
  • Practices in a professional manner and collaborates with other health team members
  • Strives to improve own level of competence through skill development and keeping abreast of social work trends and new developments in health care
  • Knowledgeable of social service procedures, policies, and resources and has the ability to analyze and interpret information to plan appropriate interventions
  • Ability to exercise judgement and sensitivity when investigating the background of clients and determining their needs
  • Communicates in a positive, professional, and effective manner with members of the health care team, clients, and their families to promote positive client outcomes
  • Answers inquiries accurately and in a timely manner
  • Writes reports, memos, and evaluations in a complete, clear, concise, and timely manner
  • Makes appropriate use of the computer in written communication and has the ability to communicate well verbally
  • Must have a valid driver’s license
  • Flexibility and willingness to change schedules and duties as needed according to staff availability
  • Willingness to travel the service area as needed
  • Must maintain a high level of professionalism, demonstrated by a positive attitude, ability to function as a team member, and a commitment to the overall mission of the agency

Nice To Haves

  • Experience in addiction counseling or child/family counseling is highly desirable

Responsibilities

  • Screen and maintain medical records
  • Conduct interviews and assessments
  • Set treatment goals
  • Arrange for expediting and coordinating services
  • Protect the confidentiality of patients and their medical records
  • Work with HCH program and staff to prepare forms, materials and protocols for integrated primary, SUD and/or mental health care services
  • Assessing and diagnosing individuals with SUD and/or mental health and disorders, as well as any other behavioral and emotional disturbances
  • Provide treatment planning and treatment implementation to HCH patients
  • Provide individual and group (including psycho-educational) counseling and problem-solving activities
  • Work with clinical staff to assist clients in achieving treatment plan goals
  • Work with a multi-disciplinary healthcare team to provide integrated care
  • Participates as a member of the Quality Improvement Committee and assists with Quality Improvement efforts
  • Performs and completes PCMH duties as assigned (attending patient huddle, involvement in QI activities and access needs)
  • Work with Medical Director for ongoing SUD and/or mental health care of patients and for coordination of emergency care
  • Work to develop SUD and/or mental health resources in the community, including increasing the availability and access of services, and support of development of 12-step and other support groups
  • Produce and/or acquire resource materials for HCH and its participants
  • Attend necessary staff trainings and team meetings
  • Participate in community activities and meetings as necessary to promote the purpose of the HCH MH/SUD Services Expansion
  • Complete program reports as assigned
  • Participate in outreach and community events
  • Perform other duties as assigned by supervisor
