Description Primary Duties and Responsibilities The Therapist provides comprehensive assessments, coordination, planning, and implementation of a psychosocial treatment plan for patients in respective department (Hoag Addiction Treatment Center – HATC, Community Health, etc.). This role promotes optimal mental health by providing ongoing psychosocial support and services to patients and their families for adjustments to illness, grief, addiction and other psychosocial stressors. Works with an interdisciplinary team to achieve treatment goals. Utilizes high-risk criteria as well as medical records, school records, clients, families, nurses, physicians and others to evaluate patient’s biopsychosocial needs. Develops a treatment plan and a comprehensive termination of treatment/discharge plan. Documents objective and accurate data regarding assessments and interventions. Maintains working knowledge of community-based resources, programs and social services. Educates patients and families on their health care options and connects them with appropriate resources. May develop and facilitate support groups, and workshops as needed. May provide input on treatment schedules, patient assignments, and/or crisis interventions as requested by Clinical Manager. Performs other duties as assigned. In addition to the above, the Therapist-Licensed may provide ongoing teaching and supervision to MSW employees and interns and may sign off on State requirements for LCSW licensing. Utilizes advanced clinical knowledge to assess and identify psychiatric and psychosocial issues facing patients. May conduct assessments related to legal ethical reporting requirements. Some departments provide additional specialist services such as Reiki Therapy when the clinician is certified to do so. Qualifications Education and Experience Master’s Degree required Working knowledge of Microsoft Office Suite required Four years’ experience in Social Work in an acute healthcare system Additional Education and Experience required vary by discipline as follows: Oncology and Palliative Care: Master’s degree in Social Work (MSW) required Experience with clinical assessment and crisis intervention required Bilingual or multilingual preferred License Required LCSW - Licensed Clinical Social Worker (LCSW), in good standing, in the State of California Oncology and Palliative Care: LCSW - Licensed Clinical Social Worker (LCSW), in good standing, in the State of California Certifications Required Palliative Care: Reiki Certification required if providing Reiki services Certifications Preferred Oncology and Palliative Care: Association of Oncology Social Work Certification (OSW-C) Advanced Palliative Hospice Social Worker Certified (APHSW-C)
