We are looking for a compassionate Hospice Social Worker to join our team. In this role, you won't just manage cases; you’ll hold hands, navigate complex emotions, and provide the "soft landing" every family needs during their hardest moments. Continuum Care Hospice, an affiliate of Legacy Health Services, is looking to hire an experienced and dedicated RN Hospice Case Manager to work with our team of professionals.
