About The Position

The Licensed Social Worker (LSW) is responsible for providing comprehensive psychosocial support to residents in a skilled nursing facility. This role ensures compliance with federal and state regulations while promoting resident well-being, dignity, and quality of life through assessment, care planning, and discharge coordination.

Requirements

Bachelor’s degree in Social Work (BSW) required
Active Licensed Social Worker (LSW) in the state of North Dakota
Previous experience in long-term care, skilled nursing, or healthcare setting preferred
Knowledge of MDS, care planning, and discharge processes preferred
Strong understanding of federal and state LTC regulations

Responsibilities

Complete comprehensive psychosocial assessments upon admission, quarterly, annually, and with significant changes
Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team (IDT)
Lead and participate in care conferences with residents and families
Coordinate discharge planning, including transitions to home, assisted living, or other levels of care
Provide counseling, emotional support, and crisis intervention to residents and families
Ensure compliance with CMS regulations and F-Tags (including F-Tag 679 – Activities, F-Tag 680 – Quality of Life, F-Tag 684 – Quality of Care)
Advocate for resident rights, dignity, and person-centered care
Assist with advance directives, guardianship, and end-of-life planning
Address and resolve resident and family concerns or grievances
Collaborate with nursing, therapy, providers, and administration to support behavioral and mental health needs
Maintain accurate and timely documentation in EMR systems (e.g., PointClickCare)
Support MDS process and documentation as needed
