About The Position

Under the direction of the Social Work Supervisor, the Licensed Social Worker (LSW) will provide general counseling to residents and their respective families and significant others on an individual or group basis. Provides advocate and direct services for residents at the Supported Living and immediate Care Facilities at I Am Boundless, Inc. Essential Functions Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. · Applies advanced knowledge and consistently exercises autonomy, discretion, and judgment and the development of comprehensive social histories and psychosocial assessments that include the resident's problems, strengths, and preferences, including implications for the care plan; helps residents and their guardians/families adjust/cope with life changes. · Provides general counseling to residents and their respective families and significant others on an individual or group basis; provides advocacy services for residents; is responsible for admissions and discharges. · Empowers residents and encourages resident choice in matters affecting them, demonstrating receptive attitudes and sensitivity to choices. · Participates in interdisciplinary team meetings with other professional staff; assesses, develops, implements, evaluates, and modifies social histories and social work service plans. · Establishes and maintains liaison relationships with various community and governmental agencies to coordinate services; conducts follow-ups on referrals and obtains information; ensures establishment and maintenance of Medicaid, social security, and other government benefits; assists in setting up guardianship; and assists in establishing needed trusts. · Reviews and monitors resident finances. · Prepares documents, correspondence, reports, forms, case summaries, etc.; maintains necessary records and files to ensure compliance with all applicable standards using electronic. · Provides in-service training programs for staff and, residents; attends staff meetings as required · Trains and consults with staff regarding needs of individual residents, the resident group as a whole, and families to increase staff sensitivity; educates staff about the Resident Bill of Rights and any relevant state legislation. · Performs other related duties as assigned.

Requirements

  • Bachelor’s degree in social work from an accredited school of social work.
  • Licensed through the State of Ohio as an LSW.
  • Ability to obtain and maintain First Aid, CPR - adult, child, and infant. (Training will be provided, if not already certified).
  • Valid Ohio Driver's License with Ohio Bureau of Motor Vehicles; proof of auto insurance.
  • No more than 5 points on license.
  • Within 60 days of hire, successful completion of the required physical crisis management training(s) and certification(s) applicable to the assigned program.
  • Must be able to function independently and have flexibility, personal integrity, and the ability to work effectively with residents, staff, and support agencies.
  • Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people within the facility.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • COVID-19 vaccine.

Nice To Haves

  • Two (2) years of supervised social work experience working directly with individuals with intellectual/developmental disabilities.
  • Licensed through the State of Ohio as a LISW.
  • Master’s Degree in social work.
