About The Position

Seeking Medical Social Worker! Full-time, 8:00am-4:30pm, Monday- Friday Baptist Health Floyd, Care Coordination Southern Indiana/Louisville, Kentucky area The Social Worker (MSW) is a non-clinical social work position responsible for discharge planning, case management, and psychosocial support of patients and their families within the hospital setting. This involves completing psychosocial assessments and interventions to assist patients in coping with their hospitalization, illness, diagnosis, and/or life situation and provide support and education to families on how to support the patient after hospitalization. The Social Worker also provides advice or guidance for any other non-medical problem or barriers to care patients may have that may hinder their rate of recovery or delay discharge and serves as a resource for continuity of care options after discharge. Current Kentucky license as a Certified Social Worker (CSW) required or Current Indiana license as a Licensed Social Worker (LSW) required based on state of practice. Requirements
