About The Position

Loyola Medicine, a member of Trinity Health, is a nationally ranked academic, quaternary care system based in Chicago's western suburbs. Loyola Medicine Transport is based at Loyola University Medical Center and is part of a three-hospital system including Loyola University Medical Center , Gottlieb Memorial Hospital , and MacNeal Hospital . The Licensed Clinical Social Worker, Transplant works as part of a multi-disciplinary transplant team, to provide individual counseling or referral for individual counseling as indicated by patient's assessment. Bi-lingual in Spanish, strongly preferred!

Requirements

  • MSW from a CSWE accredited School of Social Work in IL
  • LCSW - Licensed Clinical Social Worker State of Illinois

Nice To Haves

  • Preferred two years acute hospital or transplant experience
  • Bi-lingual in Spanish, strongly preferred!

Responsibilities

  • Assessment Conduct comprehensive psychosocial evaluations for transplant candidates and donors. Update evaluations and develop care plans based on team input. Coordinate hospital discharge planning and obtain post-op authorizations. Participate in inpatient case management and discharge processes.
  • Counseling Provide or refer for individual counseling based on assessments. Offer crisis intervention, bereavement, and advocacy support. Educate patients and families on post-transplant coping. Evaluate and refer for substance abuse and smoking cessation.
  • Program Development Collaborate with ambulatory behavioral health and providers. Organize and lead monthly support groups. Identify outreach opportunities and connect with community services. Participate in organ donor awareness initiatives.
  • Quality Engage in transplant and system-wide quality improvement efforts. Maintain documentation per UNOS, Loyola, and department policies. Ensure timely EPIC entries and accurate billing. Report potential risk or quality issues to leadership.
  • Professional Development Stay current on clinical standards, UNOS/CMS rules, and post-acute care options. Understand insurance, Medicare/Medicaid, and financial assistance programs. Adhere to ethical standards and pursue ongoing education. Mentor social work students and engage in transplant advocacy.
  • Transplant Process Knowledge Attend inpatient rounds and weekly medical review board meetings. Maintain expertise in UNOS and CMS transplant regulations.
