About The Position

Responsible for assessment and counseling and coordination of community services/referrals. Acts as an advocate for patients in the BJHCHS, Inc. health system. Maintains pertinent, accurate, timely, documentation in clinical record.

Requirements

• Master’s Degree in Social Work
• Current SC license as LISW/LCSW/LMSW
• A minimum of 3 years clinical experience providing direct services to adults, children, adolescents and families
• Minimum of 2 years case management and counseling experience
• Current knowledge of evidence-based models for mental health and substance abuse treatment
• Ability to deliver validated evidence-based therapy services to patients
• Able to develop treatment plans and document patient contact and progress
• Possesses a valid South Carolina driver’s license and clean driving record

Responsibilities

• Performs accurate assessment and treatment planning through evaluation of clinical symptoms, development and personality disorders, psychosocial stressors, and global function
• Provides effective counseling/case management services to patients
• Coordinate care of patients with healthcare providers, community agencies, specialists
• Coordinates and plans with outside agencies and home care nursing staff for post-discharge care
• Maintains timely, accurate documentation on all medical records in compliance with established guidelines and policies
• Demonstrate emotional resilience and stability
• Demonstrate excellent communication skills with patients and staff
• Capable of working with children and adults
• Performs other related duties as assigned by management
• Travels to various BJHCHS, Inc. medical center locations as needed and directed by the supervisor.
