Requirements

• Graduate of an accredited nursing program
• Current LPN license to practice in the state of Connecticut
• Proficiency in delegated nursing tasks and clinical documentation
• Valid driver’s license

Nice To Haves

• Experience working with individuals with disabilities is a plus

Responsibilities

• Provide direct nursing care, including PEG tube medications and feedings, wound care, catheter/colostomy care, nebulizer treatments, and administration rescue and routine medications (oral and topical).
• Monitor and document changes in individuals' health status, ensuring compliance with state and agency protocols.
• Collaborate with direct care staff to support daily care needs on the floor, including hygiene and ADLs, and maintain a safe, high-quality environment for residents.
