MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Now Hiring: Adult Day Center LPN (Bilingual – Cape Verdean Creole Preferred) 📍 Brockton, MA 🕒 Day Shift | No Nights or Weekends 💲 $32–$34/hour Position Summary We are seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to provide medical care and support within our Adult Day Center. This role works under the direction of an RN or physician to ensure the health, safety, and well-being of participants. Requirements

  • Current LPN license in state of residence
  • Associate’s degree in Nursing or related field (or state certification)
  • Minimum 1 year of nursing experience, preferably with adult or elderly populations
  • CPR/First Aid certification (and other state-required certifications)
  • Strong communication and organizational skills

Nice To Haves

  • Bilingual in Cape Verdean Creole preferred

Responsibilities

  • Provide basic medical care and treatment to participants
  • Complete health assessments and assist in developing care plans
  • Monitor, administer, and document medications accurately
  • Coordinate and follow up on medical and dental appointments
  • Act as a liaison with pharmacies and healthcare providers
  • Review medication errors and recommend improvements
  • Oversee staff administering medications and conduct QA checks
  • Respond to medical needs and provide emergency care as needed
  • Maintain accurate health records and ensure compliance standards
  • Train staff on medication administration, infection control, and safety practices
  • Educate participants on self-care, treatments, and wellness topics
  • Collaborate with external agencies to ensure continuity of care
  • Assist with daily activities and transportation as needed
