About The Position

Licensed Practical Nurse The Licensed Practical Nurse (LPN)monitors patient health,educates patients about disease management, and delivers long-acting injectable antipsychotic medications directly to patients in their homesand clinical settings, removing barriers to access and supporting continuity of care. This role ensures safe, timely, and compassionate administration while coordinating closely with healthcare teams. It combines clinical expertise with strong communication and organizational skills to enhance patient outcomes. Seeking a Licensed Practical Nurse (LPN) licensed in both Maryland and the District of Columbia. This role is based out of our DC pharmacy and requires regular travel to clinics throughout DC to administer injections. Candidates must be local to the DC area, as consistent travel between sites is a key component of this position.

Responsibilities

  • Administers long-acting injectable antipsychotic medications inassigned clinic locations, medical offices, or patients' homes in accordance with clinical protocols and safety standards.
  • Observes patients to assess overall wellness, address basic needs and report adverse events to the care team.
  • Coordinates with healthcare providers and care teams to ensure timely and accurate delivery of prescribed treatments.
  • Assesses patient readiness and clinical appropriateness for administration and monitors for adverse reactions.
  • Documents all visits and medication administrations accurately in the appropriate systems.
  • Provides patient education and support to promote adherence and understanding of treatment.
  • Maintains proper handling, storage, and transport of medications and supplies in compliance with regulatory guidelines.
  • Communicates any concerns or barriers to care teams to support continuity of care and patient safety.
  • Serves as a trusted advocate for patients during transitions of care.
  • Builds relationships with patients, caregivers, and healthcare teams to ensure smooth care coordination.
