About The Position
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join a well-established long-term care and rehabilitation facility. The ideal candidate will have experience providing quality patient care in a fast-paced environment, including medication administration and supporting patients with cognitive conditions.
Requirements
- Active New York (NY) LPN License (required to submit)
- Minimum 1 year of experience
- Long-Term Care (LTC) experience preferred (flexible on duration)
- First-time travelers accepted
- Local candidates only
- BLS Certification required
- COVID-19 vaccination required
Nice To Haves
- Long-Term Care (LTC) experience preferred (flexible on duration)
Responsibilities
- Administer medications accurately (Med Pass)
- Provide nursing care to residents in a long-term care setting
- Assist patients with Alzheimer’s and Dementia conditions
- Monitor patient health and report changes to the care team
- Maintain accurate patient records and documentation
- Ensure compliance with facility policies and healthcare standards
