About The Position

About You In Home Care is seeking a dedicated Licensed Practical Nurse (LPN) to oversee and support our team of CNAs and caregivers. This role is crucial in maintaining high-quality patient care, ensuring proper documentation, and upholding compliance with regulations and care plans. The LPN Supervisor will report directly to the Clinical Director and play a leadership role in daily operations.

Requirements

Current LPN license in Louisiana
Strong documentation, organizational, and leadership skills
Ability to manage multiple responsibilities and make sound clinical decisions
BLS certification required

Nice To Haves

Experience in home health, long-term care, or a supervisory role preferred

Responsibilities

Supervise and support caregivers – Provide training, mentorship, and oversight to ensure quality care and adherence to policies.
Monitor patient health – Conduct regular assessments, document changes in health status, and update care plans as needed.
Ensure compliance and accuracy in documentation – Maintain thorough records of patient care, interventions, and observations in accordance with state and company standards.
Administer medications and treatments – Ensure safe administration of medications and treatments as prescribed.
Act as a liaison – Communicate effectively with patients, families, caregivers, and healthcare providers to ensure continuity of care.
Promote a positive work environment – Foster teamwork, encourage professional development, and address any concerns among staff.
