About The Position

Provide skilled nursing care to patients in EOL, conducting visits in a facility and/or home patients. Provide on-call services after hours, and complete scheduled skilled visits. Assist Registered nurses in carrying out the plan of care. Assist RN in performing specialized procedures( as defined in the state nurse practice act) as directed by the RN under physician's orders. Actively participate in the IDGH in collaborating to ensure the patient plan of care is kept current and identified issues are addressed. Assist in the reinforcement of patient and family education regarding end-of-life care. Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on-the-job learning. Keep the RNCM informed of changes noted during the process. Participate in self-development by practicing further education, workshops, and conferences, active in professional and related organizational meetings, and individual research and reading. Perform other duties as directed

Requirements

  • Licensed Practical Nursing Degree
  • A minimum of 1 year of Clinical experience/Skilled care
  • Hospice experience preferred
  • Valid Drivers License and Auto insurance Coverage
  • Reliable transportation: Willing to travel
