About The Position

"Navitas Healthcare, LLC" is seeking a skilled Licensed Practical Nurse (LPN) to support patient care in a skilled nursing facility. The LPN will perform a wide range of nursing and clinical support activities with minimal supervision, ensuring safe, high-quality care for patients. Responsibilities include assisting physicians, monitoring patient progress, and maintaining clinical areas. Experience with Electronic Medical Records (EMR) is strongly desired.

Requirements

• Current and valid LPN license in New York.
• Graduation from an accredited LPN program.
• CPR/BLS certification required.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a fast-paced clinical environment.

Nice To Haves

• Experience in hospital, long-term care, or home health settings preferred but not required.
• Experience with Electronic Medical Records (EMR) is strongly desired.

Responsibilities

• Perform patient care functions in an acute or skilled nursing setting.
• Greet patients, prepare exam rooms, and assist during exams (e.g., setting up trays for injections, taking vital signs).
• Assist with general office duties, including reception, appointment scheduling, filing, and chart preparation.
• Coordinate with insurance companies to obtain authorizations for procedures such as PT, MRIs, CTs, and surgeries.
• Communicate with primary care physicians to obtain new referrals or extensions.
• Administer injections and assist with procedures such as pap smears.
• Maintain cleanliness and organization of exam rooms and clinical areas.
• Implement nursing care activities under the direction of the RN and report patient responses to treatments.
• Participate in nursing unit management activities.
