Licensed Practical Nurse (LPN) – Home Health In this role, you will provide high-quality skilled nursing care to patients in their homes throughout the Sarasota, FL area. Working under the physician’s plan of treatment and in collaboration with patients and caregivers, you will support recovery, promote independence, and report patient concerns or changes in condition to the RN/Case Manager. Requirements

• Graduate of an approved school of nursing
• Current LPN license in the state of Florida
• Minimum of 1 year of skilled home health experience required
• Strong clinical judgment and multitasking ability
• Knowledge of nursing best practices and confidentiality standards
• Excellent communication and interpersonal skills
• Compassionate, reliable, and able to thrive in a fast-paced environment

Responsibilities

• Participate in developing and implementing patient care plans
• Provide skilled nursing care in the home setting
• Evaluate effectiveness of care and report changes in patient condition
• Accurately document medical history and clinical progress
• Provide emotional support and patient/family education
• Communicate clearly with patients, families, and care team members
• Perform additional duties as assigned
