Incumbent is a Licensed Practical Nurse providing clinical services for clients in adult health/primary care, maternal health, family planning, communicable disease, ADAP, Ryan White and adult/childhood immunizations under the direct supervision of the assigned Senior Community Health Nurse Supervisor at the direction of the Assistant Community Health Nursing Director (ACHDON). Incumbent is expected to follow all local, state, and federal rules and regulations along with complying and performing within the scope of practice as defined by the Florida Nurse Practice Act. At the direction of the ACHDON, incumbent will provide duties at all clinics within Polk County This position serves clients that speak only Spanish, Creole, or other languages. It may require performance of interpreter duties, if bilingual, or implementation of the Department’s Limited English Proficiency Plan. As a state employee, in a County Health Department, incumbent will participate in disaster preparedness and/or recovery activities. Performs other duties as required by supervisor

  • F.S. 464 Licensed as a Practical Nurse in the State of Florida
  • Valid Florida driver’s license
  • Emergency Duty: Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.

Responsibilities

  • Interview and assist providers with clients with specific medical problems or routine health care needs.
  • Assist APRN and physician with examinations and medical procedures for clients.
  • Perform routine diagnostic tests, procedures such as taking/recording temperature, pulse, respiration and blood pressure; preparing clients for medical procedures; and collecting specimens for laboratory analysis.
  • Report any unusual symptoms of clients to clinic charge nurse, APRN, and/or physician.
  • Provide documentation in client records in accordance with Health Department standards.
  • Administers medications according to the provider’s orders. Assists in maintaining the medication inventory.
  • Maintain accurate and complete medical records.
  • When working with OB team maintains Maternal Fetal Medicine reports, referrals and high-risk OB consults.
  • Assists in the lab to process specimens, run lab reports, file lab reports, and follow-up on late/missing lab reports.
  • Assess, educate and administer adult/childhood immunizations, according to protocol for clients of all ages. Enters immunizations information into Florida Shots along with Educating patients in medication administration, adherence, side effects and contraindications.
  • Enters immunization history and immunizations given in Florida Shots and all other documentation required per Health Department policy and procedures for vaccine administration.
  • Provides screening for the HIV antibody virus, pre and post-test counsels, provides education, and refers for medical evaluation and other support services.
  • Assists in keeping medical records and maintaining inventory of medical supplies.
  • Clean work area according to clinic policy and biohazardous waste policy.
  • Assists with monitoring temperature controls with notation on appropriate logs for refrigerator and freezer per protocol
  • Assists with case management of clients and make referrals for clients needing follow-up treatment.
  • Keeps the clinic and all equipment in operating order.
  • Prepares and transports specimens to the laboratory.
  • Maintain proper storage and handling of vaccines per Health Department policy and procedures.
  • Collects and completes medical information for reporting statistics.
  • Coordinates prescription refills with pharmacies. Obtains prior authorizations for medical care.
  • Assists with issuance of ADAP medications
  • Applies nursing principles and techniques in the treatment of patients.
  • Makes assessment of patient’s condition in relationship to medication administration.
  • Ensures laboratory testing has been completed to ensure appropriate medication is being administrated in consultation with medical providers
  • Conducts health history in relationship to medication administration and adherence.
  • Attends conferences and workshops as related to HIV/AIDS.
