About The Position

Under the direction of a Provider, the Licensed Practical Nurse (LPN) provides basic nursing care to the center's patients according to all the policies and procedures, care and safety protocols, and Clinical Services Practice Guidelines. LPN to assist physician in clinic. Triage patients, assist with procedures, scheduling patients, calling patients, and prior authorizations.

Requirements

  • Successful completion of the LPN diploma program is required.
  • Current Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license is required.
  • Basic Life Support (BLS) though the American Heart Association is required.

Responsibilities

  • Identifies special needs of patients including medication and equipment requirements and communicates needs to procedural staff as well as identify and respond to patient/family educational needs and teaches in accordance with current clinical practice of standards.
  • Under the direction of an RN, contributes to the assessment of patients by collecting data for analysis by an RN.
  • Performs plan of care interventions, including specimen collection and wound care.
  • May administer medication.
  • Documents patient care given.
  • Assists RN with development of plan of care.
  • Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
  • Monitors, documents, and communicates patient's status and reactions to interventions with RN, physician, patient, and families.
  • Works as an advocate for the physical and emotional well-being of the patient.
  • Schedules appointments and procedures as ordered by the provider.
  • Obtains insurance re-authorizations as requested for testing and scheduled procedures.
  • Attends clinic meetings.
  • Maintains a clean and safe working environment.
  • Reports all incidents, hazards, drug interactions, and initiate appropriate action.
  • Completes safety instructional programs.
  • Assists other team members when time permits.
