About The Position

The Licensed Practical Nurse or Registered Nurse will evaluate our clients for emergent care needs and support our therapeutic team within patient treatment plans to include documenting the medical progress and notes while keeping up with all state and federal regulation. The LPN/RN delivers direct nursing care to include medication administration while providing support and education regarding medical issues to clients and non-medical team members. We have two PRN positions open for nights and weekends. Requirements

Valid Alabama State License-Current
At least two years of nursing experience preferred
Excellent verbal and written communication
Intermediate computer knowledge

Responsibilities

Conduct vital signs, i.e. orthostatic blood pressure, weight, height, pulse rate information on clients.
Prepare discharge planning.
Provide oversight to the Behavioral Aide working with the clients providing Basic Living Skills.
Obtain information from client regarding his/her physical status.
Make inquiries of client to include any changes with him/her, symptoms, etc.
Perform medication monitoring.
Assess medication side effects and/or adverse reactions.
Perform crisis intervention techniques as needed.
Administer injections, document in client's electronic medical record on a regular basis.
Comply with regulations when administering I.M. medications.
Interface with local laboratories to obtain test results.
May be asked to work at other WellStone sites.
Maintain licensure in the State of Alabama.
Complete and maintain documentation in a timely manner.
Complete other duties as assigned.
