About The Position

At SBELIH, the Licensed Practical Nurse (LPN) performs tasks and responsibilities within the framework of case finding, health teaching, health counseling and provision of supporting and restorative care under the direction and supervision of a Registered Nurse (RN). The LPN is responsible, under the direction and supervision of an RN, for execution of the medial regimens prescribed by the licensed independent providers authorized at SBELIH. The LPN is responsible for the care of patients throughout the lifespan from admission to discharge. Duties will include administering prescribed medications and treatments, ensuring accurate and thorough documentation, and performing direct patient care within their scope of practice. The LPN will provide preceptorship to LPN nursing staff orienting to the assigned unit.

Requirements

  • Must possess or be eligible to obtain a license to practice as a Licensed Practical Nurse in New York State.
  • Basic Life Support ("BLS") certification required.

Nice To Haves

  • Acute care, phlebotomy, EKG, and/or IV experience preferred.

Responsibilities

  • Collects data through observation and communicates information to assist the RN and/or provider in patient assessment and care planning.
  • Participates as a member of the health care team and accepts delegation from the RN/assigned supervisor and provider in meeting needs of the patient/family.
  • Delivers care in accordance with the patient care plan, policies and procedures of the organization and principles of relationship-based care.
  • Possesses excellent communication skills; is skillful in mentoring and teaching and participates on committees or projects.
  • Plans and manages patient care according to each patient's needs.
  • Interviews patients and records their medical history and physical condition.
  • Obtains patient vital signs, including pulse, blood pressure, temperature and respiration.
  • Provides routine care for patients.
  • Observes patients' health.
  • Revies medical charts.
  • Accompanies patients to their rooms.
  • Assists physicians and RNs.
  • Communicates with patients and their families.
  • Performs standard diagnostic procedures including EKG, phlebotomy, venipuncture, PFT and Doppler, depending on competency.
  • Prepares specimens for transport to lab.
  • Administers medications.
  • Provides wound care.
  • Prepares and administers injections and/or enemas.
  • Assists with patient comfort.
  • Monitors catheters.
