About The Position
As a Licensed Practical Nurse (LPN) , you will be joining a team of nursing professionals dedicated to providing excellent clinical care. Some have been with us for a long time and can mentor others. You will help patients reach their clinical goals and work side by side with a highly skilled multidisciplinary team. Our Floor Nurses are among the best & they would love to have you join them. New Grads are encouraged to apply Our patients are loud, happy and eager to say hello to you as you pass by. Our population is about 50% skilled and 50% behavioral health – giving you an opportunity to work with a variety patients and expand your knowledge, all in one center. This is a large, inner-city skilled nursing facility (SNF) that provides care across multiple levels—from geriatric long-term care to a robust behavioral health program. Our patients range from their mid-40s to 80+, many living with physical, cognitive, or behavioral health conditions. This center is managed by TRANSITIONAL CARE MANAGEMENT. We currently provide comprehensive management services to several inpatient healthcare centers throughout Northern IL & the Chicagoland area. These settings include Long-term Care Skilled Nursing (SNFs), Short-term Care Rehabilitation, Intermediate Care (ICFs), and Behavioral Health Service (SMHRFs) centers. Each center is unique; offering you a variety of long-term career opportunities.
Requirements
- Licensed Practical Nurse (LPN) in IL
- Electronic medical records experience is necessary (Point Click Care or PCC is ideal)
Nice To Haves
- Prior floor nurse experience within a skilled nursing facility is helpful
- Experience with the behavioral health population is helpful, but not required
