About The Position

"Navitas Healthcare, LLC" is a compassionate Licensed Practical Nurse (LPN) to provide direct patient care in a Long-Term Care setting. The role involves supporting residents’ health, comfort, and safety while collaborating with the interdisciplinary care team.

Requirements

• Active LPN license in Illinois (required)
• Strong clinical judgment and communication skills
• Ability to work 12-hour shifts in a fast-paced environment

Nice To Haves

• LTC or skilled nursing experience preferred
• BLS certification preferred

Responsibilities

• Provide direct nursing care to LTC residents
• Administer medications and treatments as prescribed
• Monitor and document patient conditions and report changes
• Assist residents with ADLs and mobility support
• Collaborate with RNs, physicians, and care teams
• Maintain accurate clinical documentation
• Ensure compliance with facility policies and safety standards
• Respond promptly to patient needs and emergencies
