About The Position

The Licensed Practical Nurse (LPN) in a long-term care setting provides direct patient care to residents, ensuring their safety, comfort, and overall well-being. This role works under the supervision of a Registered Nurse and collaborates with the healthcare team to deliver high-quality, patient-centered care.

Requirements

  • Diploma or certification from an accredited Practical Nursing program or High School Diploma
  • Active LPN/LVN License
  • Basic Life Support (BLS) certification – required
  • Minimum 1 year of LPN experience in long-term care or skilled nursing facility preferred
  • Familiarity with EMR systems
  • Ability to work flexible shifts, including weekends and holidays

Responsibilities

  • Provide direct nursing care to residents in a long-term care facility
  • Administer medications and treatments as prescribed
  • Monitor and record vital signs and patient conditions
  • Assist residents with activities of daily living (ADLs)
  • Perform wound care and other clinical procedures
  • Maintain accurate and timely patient documentation
  • Report changes in patient condition to the RN or physician
  • Ensure compliance with infection control and safety protocols
  • Support care plan implementation and updates
  • Respond promptly to patient needs and emergencies
