About The Position

This position will work as a member of a multi-disciplinary team to deliver comprehensive, person-centered, integrated services to the community with a focus on medication tracking, ordering, handling and auditing. The LPN plays a critical role in medication management and patient care by supporting providers and clients and are also integral members of the treatment team. LPNs work with client access specialists, and non-clinical staff to ensure the workflows are efficient, the client experience is positive and medication safety and accuracy is prioritized. Additionally, this role includes interaction with clients during medication pass times, offers general assistance with medication self-administration, supports medical issues or concerns as a key member of the integrated treatment team. This position has access to mental health billing and service records. The Burnside Recovery Center offers 3.7 Level of Care services, focusing on stabilization and support for clients with substance use disorders as their primary diagnosis, along with co-occurring mental health conditions. Our integrated, person-centered care model meets clients and their families where they are, combining therapy, mental health services, medical care, and community-based management into one coordinated pathway to recovery. With expedited access to primary care, outpatient substance use disorder services, and recovery-supportive housing, our holistic approach empowers clients to achieve long-term success. Requirements

Completion of an accredited course of training for licensed practical nurses.
Current licensure with the Oregon State Board of Nursing as a Licensed Practical Nurse
Must meet CCC privileging requirements as required by FTCA
Current BLS CPR certification prior to start date.
Ability to adhere to Central City Concern’s drug-free workplace which encourages a safe, healthy and productive work environment and strictly complies with the Drug-Free Work Place Act of 1988. Must have the ability to read labels and distinguish between colors.

Must pass a pre-employment drug screen, TB test, and background check. This includes clearance by the DHS Background Check Unit.
If driving, must possess a current driver’s license, access to a vehicle, quality as an Acceptable Driver as designated in CCC Fleet Safety.
Must pass an initial drivers training within 60 days of being an approved driver and continued recertification training.
Must maintain vehicle insurance coverage of a minimum of $100,000/$300,000 personal auto liability coverage.

Responsibilities

Work as a member of the 16th and Burnside multi-disciplinary team to deliver comprehensive, client centered, and trauma informed integrated services.
Actively participate in the integration of services and identify new opportunities to improve collaboration between and within service and care teams while carefully following information-sharing guidelines.
Proactively identify opportunities for improvement within workflows and logistics.
Participate in review of Key Performance Indicators; review and contribute to continuous improvement opportunities and plans.
Build understanding of and appreciation for diversity among clients and support their connection to culturally responsive and culturally specific services as requested.
Track, audit, handle and order medications within the LPN scope.
Take primary accountability and coordinate the tracking, auditing and ordering of medications upon admission and during a client’s stay in the facility.
Work under the direction of a Licensed Medical Provider (LMP) and RN regarding medication orders and treatments and/or plan of care.
Evaluate response to focused plan of care interventions as directed by RN/LMP.
Communicate client’s status, and any concerns or issues regarding RN/LMP plan of care, medication or treatments with the RN or LMP providing clinical direction per the context of care.
For any situations that present outside of an RN or LMP direction, collect data and seek direction from provider or RN.
Assist the medical assistants and RNs as needs arise and as directed including communication with outside medical partners, internal and external referrals, stocking, client education and rooming clients.
Perform activities and interventions within LPN scope of practice and individual scope of practice (i.e. LPN has been trained and demonstrates competency).
Assist clients with following medication self-administration directions
Do regular quality audits of the MAR in consult with medical staff
Create new Medication Activity Record (MAR) or change MAR as directed by LMP orders
Keep client medication bins organized and up to date
Coordinate with RNs/LMP to clarify medication orders
Provide daily feedback to the medical team on client medication concerns, MAR discrepancies, reported medication needs, etc.
Facilitate medication self-administration in-room for clients on isolation protocol
Follow controlled medication handling, including methadone, and med destruction protocols
Take primary accountability for pharmacy communication and coordination.
Maintain medical/lab equipment and records in compliance with regulatory requirements.
Maintain and clean supply rooms, exam rooms and MA workstations.
Receive, document and direct client telephone calls in a complete and timely manner according to protocols.
Outreach to clients to encourage engagement in care and proactive health measures
Complete and document procedures, medications and treatments per provider orders and clinic standards
Maintain a clean, safe clinic environment, respecting scope of practice and clinic best-practice.
Observe clients of the same gender as they provide urine drug samples, secure samples via chain of custody procedures.
Maintain and complete appropriate data entry as it relates to urine drug screens, such as transcribing point of service results as well as lab results.
Ensure that urine drug screen procedure is trauma informed, and patient centered.
Administer medications and treatments as directed by the standing orders and/or by LMP
Follows individualized client care plans
Respond in emergency situations.
Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security.
Support compliance with all privacy and security requirements pursuant to community partners’ and outside providers’ patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.
